With multiple provider designations, we furnish service based on the needs of you and your residents while always respecting patient choice. Moreover, our extensive service offerings expand with expected volume, giving us the ability the further develop innovative programs such as our On-Site Outpatient Therapy Clinic, Resident Coordinator, Value Based Analytics, and Remote Therapy Monitoring. Communication and Care Coordination are key.  We make it easy.

Skilled Home Health

Often, a resident’s ability to remain or return to your facility is enhanced by available skilled care. We can provide all the skilled care your residents require, including, Medicare Certified Skilled Nursing, Physical Therapy, Occupational Therapy and Speech Therapy. 

On-Site Outpatient Therapy Clinic

Using available space we’ll lease from you under Safe Harbor Guidelines, we will install a fully equipped Outpatient Clinic with all the tools your residents need to reach their maximum potential. We bring 30+ years of Medicare Certified outpatient clinic experience right inside your door, giving you a unique marketing edge and your residents unmatched access and convenience.

Continuity of Care

Transitions can be difficult, but with Marden your residents will move seamlessly from Medicare Part A to Part B services, with the therapists they know and trust. These continuing services create continuity that truly supports your aging-in-place model.

Full Time PTA Manager

With our PTA Manager, we are continuously available to meet your needs. Communication and Care Coordination are the primary attributes of our managers in addition to daily treatments. Their focus is on the well-being of your facility and its residents while coordinating our care with your staff.

Resident Coordinator

In certain instances, and under Safe Harbor Guidelines, we’re able to enhance our services with our exclusive Resident Coordinator, an additional half-time, non-treating professional. Duties can include:

• Exercise Classes up to 5 days per week

• Resident Education Programs and Activities

• Staff Education Program and In-services

• The Resident Coordinator brings “Supercharged” Total Service.

Marketing Support

At your request, we’ll be part of your new resident marketing. Our manager will review our services while engaging with the potential resident to find out what they really need. We’ll reassure them that we’re genuinely interested in improving their well-being.  Our On-Site Clinic will leave them with a lasting impression of the available extensive care services.

Staff In-Services & Specialty Programs

As part of your services and under Safe Harbor Guidelines, we’ll provide continuing staff in-services and specialty approaches tailored around your identified and requested needs. These can include various health screenings, fall reduction strategies, reducing hospital readmissions, and disease management.

DME Management and Lifeline Monitoring

We are your source for Lifeline Medical Alert Services, so your residents are never truly alone. Let us review the options and find the system that is right for them.

Additionally, we will assist your residents in ordering their Durable Medical Equipment such as wheelchairs, walkers, and hospital beds.

Marden Solutions™ ILF Services

If you have an Independent Living Facility on your campus in proximity to an ALF we service, in addition to our Skilled Home Health, we can furnish a menu of Private Duty care options designed to make your residents’ lives a little bit easier. Our options are tailored for the needs of the residents we serve and are adjusted from time to time based on demand. Reimbursement for these services is exclusively through Private Pay.

These are the standard services provided by Marden. They may vary by location based on our local volume and current staffing levels. Our billing is monthly with all services on the same invoice, conveniently paid by an automatic debit from your bank account or credit card. Private Duty Services are not generally covered by Medicare or insurances. Private Duty Services are typically available between 8 AM and 4 PM weekdays.

Medication Management

Our Medication Management Services are designed to assist you in properly managing your medications. It consists of several required components, and under our approach, your medication stays with you.

Bathing Assistance

Bathing Assistance is provided in 15-minute units as your assistance level requires. For dressing and other Activities of Daily Living assistance, see Therapy Consultation.

Continuity of Care

Our nursing staff will provide consultation for your medications and other care needs. As example, they can assist with education and medical management requirements concerning CHF, Diabetes, Asthma, use of Inhalers, etc.

Therapy Consultation

Our therapy staff will provide consultation for assistance with Activities of Daily Living as well as individualized exercise and conditioning programs.

Call Reminders

We will arrange individual reminder calls through our automated system starting at less than $1 per day. We can provide personal reminders of what med to take when and contact a loved one if you don’t respond.

Special Assistance

Our special assistance services are available on a case-by-case basis. Generally, minimum service is for 15 minutes. However, there are times when a quick task only takes a few minutes. We can help there too.

Trash Removal

For those of you who have selected other Marden services including Home Health, we will place your trash in the building’s trash chutes at the close of our visit.
